Service Agreement
Payment of fees are due and payable in advance. Checks should be made payable to Jan's Pet Sitting.

In the event of an emergency, we will attempt to contact you and/or your veterinarian. In the event contact cannot be made with you or your veterinarian, your pet will be taken to a nearby veterinarian or to the closest emergency veterinary clinic. In the event of a veterinary emergency, I hereby give permission to Jan's Pet Sitting to seek medical attention for my pet(s). I release Jan's Pet Sitting from all liabilities related to transportation, treatment, and expense. I understand I will be responsible for payment of any and all charges incurred.

In the event Jan's Pet Sitting performs additional services, Jan's Pet Sitting will bill you at our hourly rate. Additional services include, but are not limited to, transportation to veterinary/emergency veterinary clinic and trips to purchase needed pet related supplies. This bill will be due and payable upon receipt. You will be billed for travel time as well as “wait” time, as well as for any supplies that have been purchased.

If my pet is allowed free access to the outdoors, I hereby release Jan's Pet Sitting from any liability in the event of injury, death, or disappearance of my pet, and I will assume any responsibility for any damage or destruction caused by my pet.

If the client's pet(s) bites or otherwise exposes the pet sitter of Jan's Pet Sitting to any disease or ailment during the performance of the job, the client will be responsible for any and all medical costs and damages incurred by the pet sitter of Jan's Pet Sitting.

Jan's Pet Sitting shall not be held responsible for any damage caused by third parties who have access to client's home. Such third parties include, but are not limited to, cleaning and service personnel, neighbors, relatives, etc. Client agrees to hold Jan's Pet Sitting harmless from and against any and all losses, injuries or claims which arise as a result of the negligent act or questionable act of any party not contracted by Jan's Pet Sitting. The client waives any claims against Jan's Pet Sitting unless Jan's Pet Sitting is negligent and does not perform as agreed herein.

I hereby give my permission to Jan's Pet Sitting to enter my premises at the above address for the purpose of providing pet care for this and any future pet care services we request from Jan's Pet Sitting.

I have read and agree to the terms of this Agreement.
